There are several scenes in which the boy defends himself from the spirit, which sometimes takes human form, and these scenes include him stabbing it, spraying pepper spray in its face and slashing its Achilles tendons when it's in the form of a woman. In one scene a character breaks another character's foot by twisting it 180 degrees. Characters strangle and choke others in some scenes. There is also strong violence in repeated flashback scenes when a character takes their own life by putting a pistol to their head and pulling the trigger, with brief sight of blood.

There is sustained supernatural threat throughout the film as the spirit stalks and terrorises the boy across the apartment. In some scenes it appears as a grinning beast, and in others it looks like a woman with large sharp teeth.
In one scene the boy pricks his finger with a pin and drips the blood into a candle as part of casting a spell.
