BULLETSTORM is a science fiction action game played from the first-person perspective. The player assumes the role of Grayson, a tough soldier who plunges both himself and his team into mortal danger when they break free of their commanding officer. The game was classified '18' and contains frequent strong bloody violence and strong language.

At '15' the BBFC's Guidelines state that 'Violence may be strong but should not dwell on the infliction of pain or injury'. During BULLETSTORM there is frequent strong bloody violence. Blood splashes onto the screen, heads are decapitated and limbs are blown off in chunks as the player's character shoots enemies with a range of weaponry. New weapons and upgrades to existing weapons (such as sniper rifles and machine guns) are available, while a Leash weapon allows the player to throw enemies high into the air, allowing them to be shot at will as they slowly fall back down to the ground. If the leash is used to pull an enemy towards the player, their bodies can be impaled on scenery, such as spikes in the environments. Such violence is presented in an arcade game style, with an over-the-top atmosphere throughout. Points are awarded for head shots, ingenious methods of causing death and destruction and so on - for example when an enemy is blasted into a gate and explodes into pieces, a bonus is awarded. Some of the game's environments also feature blood smeared on the walls, characters impaled on spikes, etc. This focus on bloody injury and violence was considered to go beyond the '15' Guidelines, and so the game was classified '18'.

There is strong language present throughout the game. Other language includes uses of 'shit', 'whore' and 'retards'.
